When it was accidentally approved, a rift in the space time continuum was created and an evil goateed version of the pilot was created 24 hours in the past. Before the leave monitor could correct the mistake, the evil pilot had engaged the good pilot in aerial combat.

As he was maneuvering for a kill, the lack of protective, fireproof, and ultimately sweat soaking material caused his hands to slip from the throttles and stick and land on the ejection handles.
