 Good Morning! My name is Michael Velasco, I am a senior at NeoCity Academy and for my AP research class I am doing a survey to study the correlation between a pilots drinking/smoking habits and am hoping to gather participants through this forum. The information on the survey will be kept completely confidential and an email will only be used to make sure every participant only fills out the survey once.

Oh, I am sorry about that. The study is on the correlation between a pilots drinking/smoking habits and their overall Job satisfaction. I apologize for the confusion. I hope this makes more sense!

For all those participating in this survey- In the Google doc before the survey is taken there is a simple consent form that assures you that all the information gathered in this survey will be kept COMPLETELY confidential. No names, emails, or other information regarding the identity of the responses will be shared with the data in the final research paper!
